With entering into our patient initial admission/enrollment your data is being processed by the practice consent to E.G.T. RAMSTEIN, Am neuen Markt 2, 66877 Ramstein-Miesenbach / PHYSIO ELITE, Kerststr. 27-31, 67655 Kaiserslautern / PHYSIO ATHLETICS, Brüsseler Str. 7, 67657 Kaiserslautern, may be stored and processed for the following purposes.

Purpose: treatment and billing for our treatment and therapy facilities.

To relieve our administration, our bills for the therapy services you have used are processed by the respective billing center of your health insurance company. 

We therefore ask you to give us your consent for the transmission of all personal data necessary for billing purposes by means of your signature. The transmission includes, in particular, address, date of birth, cost bearer, diagnoses, date of treatment and therapy measures provided. 

I consent to the above mentioned practice transferring patient data to the respective companies for the above-mentioned purposes within the scope of the necessity. I am making this declaration voluntarily. I know that I can revoke this consent at any time with future effect without giving reasons.

We, PHYSIO ELITE, Kerststr. 27-31, 67655 Kaiserslautern, PHYSIO ATHLETICS, Brüsseler Str. 7, 76757 Kaiserslautern, E.G.T. Ramstein, Am neuen Markt 2, 66877 Ramstein-Miesenbach with the therapists, back office staff and trainers and all relevant or specific therapists and staff members. 

For this purpose, the necessary data will be forwarded to the named bodies, which will process it for the following purpose/measures: Implementation / continuation of the therapeutic service and organizational and administrative processes. 

The following data is affected by you: 

name, address, date of birth, cost bearer, diagnosis, date of treatment and therapy measures provided. 

I know that I issue them voluntarily. I have taken note of the fact that I can revoke this consent at any time without giving reasons with effect for the future. The revocation must be made in the practice mentioned above.
